Required Forms & Safety Policies

Registered Massage Therapy in Ontario requires the collection of relevant health information to ensure safe and appropriate care.

All appointments require:

• Completed health history & intake forms

• Scar history disclosure (where applicable)

• Sensitive area consent (if applicable)

• Intra-oral consent (TMJD care, if applicable)

Important

• Forms must be completed before the appointment

• Treatment cannot proceed without completed documentation.

• Treatment is provided only to the individual who completed the intake and consent forms. Substitutions are not permitted.

• This applies to all patients, every visit

Prepayment & Cancellation

• Prepayment is required at the time of booking

• Cancellation timelines and fees are disclosed in advance

• Missed or incomplete requirements are treated according to policy

• All information is confidential and protected under PHIPA.
