Why do I have to complete forms before treatment?

Ontario law (RHPA / CMTO) requires Registered Massage Therapists to collect relevant health information to provide safe care.

Treatment cannot proceed without completed forms.

What if I don’t want to answer certain questions?

You may ask why information is requested. Some information is required for safety. If required information is missing, treatment may need to be deferred.

Is my information confidential?

Yes. Personal health information is protected under PHIPA and kept confidential.
