Why do I have to complete forms before treatment?

Registered Massage Therapists in Ontario are required to gather relevant health information to provide safe, effective care.

These forms help your therapist understand your health history, current concerns, and any factors that may affect treatment — so your session can be comfortable and properly paced.

For safety reasons, treatment can only begin once required forms are completed.

What if I’m unsure about answering certain questions?

You’re always welcome to ask why specific information is requested — transparency is part of good care.

Some questions relate directly to treatment planning and safety, such as:

• recent surgeries or procedures

• implanted medical devices (such as pins, plates, screws, or wires)

• current medical conditions or medications

• healing tissues or movement restrictions

These details can influence positioning, pressure levels, comfort, and whether certain techniques are appropriate.

In some cases, missing safety-related information may mean treatment needs to be adjusted or postponed to ensure your wellbeing.

Is my information confidential?

Yes. Your personal health information is protected under Ontario’s PHIPA legislation and is kept secure and confidential at all times.
