Why are forms completed before treatment?

Registered Massage Therapists in Ontario are required to gather relevant health information to provide safe, appropriate care.

These forms help your therapist understand your health history and any factors that may influence treatment, so each session can be planned thoughtfully and comfortably.

For professional and safety reasons, treatment can begin once required forms are completed.

What if I’m unsure about answering certain questions?

You’re always welcome to ask why specific information is requested — transparency is part of good care.

Some details are important for safety and treatment planning. In situations where essential information is missing, care may need to be adjusted or postponed to ensure your wellbeing.

Is my information kept confidential?

Yes. Your personal health information is protected under Ontario’s PHIPA legislation and is handled with care and confidentiality at all times.
